Start here
- Confirm the current providers, their roles and the date any existing arrangement ends.
- Ask whether the receiving provider has accepted responsibility; a sent referral is not confirmation.
- Agree relevant information sharing through provider procedures and identify who will follow up outstanding actions.
- Confirm the next appointment, funding, actual contact hours and appropriate local urgent pathways.
Relocation or living between countries
- List the locations relevant to upcoming care without sharing an unnecessary travel history.
- Ask each clinician what they can provide in the actual destination, including online appointments.
- Confirm local access, accepted referrals and responsibilities during the interval.
- Ask qualified professionals about any treatment-specific travel requirements.
University or study abroad
- Ask the student what support and family involvement they want.
- Confirm university, community and existing clinical services separately.
- Check arrangements across term time, holidays and changes of address.
- Discuss any transition between child and adult services with the responsible professionals.
Tour or production
- Check locations, time zones, private appointment space and schedule changes.
- Agree the manager or production team’s limited logistical role.
- Separate project funding from any expected reporting about care.
- Confirm what continues after the project and who arranges the next handover.
Changing therapist or psychiatrist
- Define the reason for changing and what the new relationship needs to provide.
- Confirm the new provider’s scope, acceptance and initial appointment.
- Ask the responsible clinicians about continuity of any current treatment or prescribing.
- Update administrative contacts and permissions after the handover is agreed.
Returning home after treatment
- Ask the treating team to explain the discharge and follow-up arrangements.
- Confirm that community providers have accepted the proposed roles and appointments.
- Agree practical support at home with the person and the people providing it.
- Keep clinical instructions in the professional team’s authoritative form and arrange an appropriate review.
