- Ask the provider to explain confidentiality, its relevant exceptions and the process for recording or changing permissions.
- For each arrangement below, discuss the purpose, recipient, information, channel and review point.

Direct contact with me
- Which email, phone or provider portal should be used for different kinds of communication?
- Is voicemail appropriate, and what will appear in reminders or calendar entries?
- How can I speak directly with the clinician if someone else manages scheduling?
Assistant or professional representative

- What information is necessary for those tasks, and what should remain in direct clinical communication?

- Is any report being commissioned, for what purpose and with which recipients?
- How are funding, clinical decisions and information-sharing arrangements kept explicit?
Communication between clinicians
- What does each clinician need to know for their accepted role?
- Who organises an authorised exchange and confirms that it has happened?
- Which records stay with providers, and what belongs in a limited operational summary?
